Physician's Order Sheets DPO3

Usage: Allows three days of orders. Physician writes first day's orders. Part one is lifted from the bottom to allow removal of Parts 2 & 3 at the first perforation. Parts 2 & 3 are glued at left and perforated to form three 8 ½ x 3" sets, which can be easily removed as a set and separated for distribution to Nursing and Pharmacy. Process repeated for second and third day's orders.

Special Design Features:

• Two copies of each day's orders: the special glue feature allows Pts. 2 & 3 to form three individual 2-part (8 ½ x 3) sets – to be distributed (1) to Nursing without delay for transcribing, and (2) to Pharmacy.
